there are some websites which provide this service the best and trusted one i know is bit-x  you can check their rates at their website there are also some reviews at their thread checkout services announcment section

There are a few bitcoin debit cards out there, Xapo, Bit-X, I believe Bitstamp also provides a debit card, Coinkite...

I say a topic somewhere in the forum with an overview of bitcoin debit cards.

There is few , there is Bit-x.com which they removed the monthly fees or something if I remember right . there is also Advcash which is most likely the cheapest out there but require verification , personally I used real documents but go denied  Undecided

If you want a debit card try to go to xapo,bit-x or advcash
I use advcash Virtual debit card since most of my shopping is done through internet and i can say it works great Smiley
The only problem with advcash is its big deposit fee :/

some exchanges allow you to withdraw exchanged btc $ to your personal debit/credit card so you do not have to order card for that purpose
