I've written a fairly in depth blog posting which details a few real life issues that Bittylicious has been having concerning the transaction malleability issue.

The main focus on this is about unconfirmed change outputs. This is generally known by the devs, but I think not that well known by some Bitcoin operators or wallet providers, and probably quite interesting to a lot of users too.

Have a read at:

https://blog.bittylicious.com/2015/10/transaction-malleability/

It was written partly for myself, to get my head around this issue, and also for others to hopefully learn from. I intend on updating it when we resolve the issue in there about a few transactions that have already been sent.
