Many guides suggest that this does not scale well for larger uses. I.e. a few hundred or thousand accounts. Is this still the case?
Is it better to just exclusively use app database and ignore account feature?

Transactions must be included in a block to be properly completed. When you send a transaction, it is broadcast to miners. Miners can then optionally include it in their next blocks. Miners will be more inclined to include your transaction if it has a higher transaction fee.

The account feature in Bitcoin Core generally does not work the way most people seem to expect it to.  Most of the time you are much better off setting up your own account database.
