Looks like there isn't a ready-made form (I presume the one provided by ledger is not available in digital form, so you most likely won't find a copy of that one).
Time to start up the word processor of your choice and get creative! Recreating a form like that isn't really hard, you could probably do it in less time than it took me to look for the file on the internet and write this response.
Alternatively, for 0.01 BTC I'll design one for you (PDF file, you need to print it out yourself)  Grin

Why don't you just find a nice picture on the internet of it in high quality. Then find nice cardboard at a stationary shop. Or just design you own one. Not a big mission.

Yea thats right, print that, and take an older book with hardcover, detach the hardcover from the book and then glue it to the back side of the paper.

Then you will have a solid sheet that wont tear apart.

You can also look at something such as laminate it to make it more water resistant if not proof.   It is kinda preparing for something that will never happen.  Also I suggest making more then 1.  They should be stored securely but having just one or in one spot is a bad idea as one point of failure.

Also I am kinda paranoid when I print one.  I make sure to run virus/malaware scans beforehand.  I also clear the cache of the printer.  It is something the dumber the printer you use really the better as you don't want it being in a cache or stored for any amount of time past printing.

You could add recovery phrase near private key.

But i think you are looking this specific design u mentioned to print coz you like it Smiley

I would rather have it written down somewhere safe than never writing it down.

I never said store it at home in your sock draw, depending on how many coins are held in such address you may be wise to keep it in a safty deposit box.
If not a lot of coins just keep it in a small safe at home, but always keep your wallet seed no matter how paranoid you are.

I made copies of the sheet that came with my Nano S.

I'm looking into steganographic solutions for storing the written seed words but the problem is they have to be computer generated so I would have to set up a secure PC just to be safe.

What if someone came into your house and stole your TV? What if someone stole your whole PC? What if a person you know sniffs around and finds your jewelry or gold? What if your ex mate or spouse or some for grabs your car keys and steals your car?

Seriously dude wake up please. Do you think bitcoin is the only thing people can steal? Who would go snoopng stound your house for so long they find your private key and why would they even look at s piece of paper to begin with? Why would they even find it don't you keep it safe? Do they even know what a private key is?

Burglars don't break into people's houses looking for Bitcoin private keys they there to take your TV or gaming consoles, jewelry and anything else of value. They bit looking for a piece of paper.

Why would any of this even happen? You should have your private key in a safe place hidden away or in a safe.

One nice way is to make a QR-code from it. It is easy, but you should use a program that is on your own computer and not QR-code generator from a web-page.

For example in Linux You can install "qrencode"
and make a QR-code using this command:
