Is pywallet still useful? I need to manually add a passphrase to a .dat wallet but can't do it from bitcoin-qt or any program like that. I also need to extract priv keys from it as well.

You can add a password with bitcoin core, same for extracting private keys. IMHO pywallet still has its uses, but only in a few corner cases as most of the functions it used to provide are no build into bitcoin core.

Im not really here, its just your imagination.
