Has anyone developed a bitcoin bank that issues visa/mastercard debt cards yet? Where you would hold bitcoins in that account and when you use the card at a retailer it would just sell your bitcoins on the exchange for usd after the transaction has been processed.

What are the most liquid ways from bitcoin -> usd.

OKPay issues a mastercard debit card linked to an OKPay account which can be fueled with BTCs. Unfortunately they stopped accepting BTC recently, but they say they'll resume accepting it soon.
