the polish cards are 3 dollars for an ATM withdrawal, free to load. I don't know what more you could ask for when going btc to cash.
Yes but usually <$200 max withdrawals so that $3 is almost 2% each time, plus the ATM's own fees... adds up
1.5%, most ATMs do not have a fee. Wells Fargo has a $5 fee but I use a keybank ATM and there are no fees.

The card itself will cost you about $35 and gives you $3,500 per year. You can buy as many of them as you want.

Even if you find a way that is magically 1% cheaper, I don't know why you would rather do stuff over the mail or with other people when you can just do it anonymously with a card.
