Each block is stacked on top of the previous one. Adding another block to the top makes all lower blocks more difficult to remove: there is more "weight" above each block. A transaction in a block 6 blocks deep (6 confirmations) will be very difficult to remove.

You shouldn't use your passwords on unsecure devices. That is the biggest reason.

Carelessness of course. Being too lenient with your password also. Though i don't think you can get away from it if the hacker is really bent on accessing your acct. Just make sure that you don't let anyone get to your personal information. There's nothing wrong with making precations

1. Password being to easy
2. Being Irresponsible

Most people are hacked due that reason I stated above. The password being easy is just really a bad security for you account. Being irresponsible like using a not secured network and just connecting anywhere just to have an access to the internet is downright stupid. Also, posting on social media the places you are going, the places you've been and posting most of your information online.

Most people I've read about being hacked had an obvious security breach. Make sure to use different passwords for your email and all websites. Have 2FA activated on exchanges. Have up to date virus protections. Try not to use the same device that you keep your wallets and coins on for viewing things like porn or other questionable websites.

It also doesn't hurt to invest in better wallet security like a hardware wallet. Never share information online about your holdings to prevent becoming a target.
