They are usually per transaction but I'm unsure if this applies in this case.

There is no single transaction requirement. You can make separate contributoins on the ico for this.

The hard cap is reached when 120 million GUESS tokens are sold out. There is no soft cap at the moment since pre-ICO phase raised 872 ETH, already.

On this regard, there are levels which are way more worth of consideration than others.
