You need to fill up the form right after you have done the activity.
For Example: You tweet, share or Like any post now, then please fill the form immediately & post links to the activity you have done.

Regarding your missing Bounty, I have checked and updated it. It was not removed, you had put a link to Bitcoin profile, so it was listed under that. Please always put facebook profile name/twitter handle in this field: "Enter your Bitcointalk or Social Media profile name"
