The gap between the rule-breaking and bans are simply because of how humans work. Mods aren't machines and they can't monitor posts as they come out. For copying and pasting, users would be confused because the ban came out of nowhere with no explanation - if they're copying and pasting in the first place I wouldn't think that they would realize it was the problem. Or if they did then they wouldn't have made a thread about it: think of it like survivor bias. In terms of insubstantial posting, that is through many reports of the same user [IIRC] and thus the ban is set out some time after the postings have been made.
Most of the threads come down to the fact that the user creating them doesn't fully comprehend the rules and isn't aware of the fact that spam is bad.
