CampBX in the USA is really good.  They accept USPS money orders and Dwolla as well.

I know BitInstant is extremely fast. You get your coins in about an hour tops. I don't know their max purchase size limit, however. Check out their site. Cheers!

It depends on how (and how much) you want to pay for them.

If you are in the UK, BitBargain works instantly (you might want to try first with smaller amounts). Or in general otc I guess.
