Can i just switch back to BTC and we are "even" or should you do something to reset the balance to zero?

If you switch back to BTC and bring your balance back to a positive value that should be the same as paying back manually. If you have received a BTC payout on top of that you could send it back to the address you got it from.

Ahh, no, I don't think we'll add Italy. We are having enough trouble fighting off abusers in the countries we already have.

It doesn't work because your need to send a Caller ID otherwise your calls are rejected, and VOIPs, specially free ones, usually don't transmit this.

Netherlands seems to be working fine for many users. Many calls at the same time. Probably your provider blocked you.
