Lists are updated at both places here on the main post and on the website too. Let me know if you find any error on the list or have any suggestion.
I am not adding my referral link here on the forum as it is not allowed/it looks spam too. But you can find my referral link on my website.
Each block is stacked on top of the previous one. Adding another block to the top makes all lower blocks more difficult to remove: there is more "weight" above each block. A transaction in a block 6 blocks deep (6 confirmations) will be very difficult to remove.

You only listed few exchanges while we have a lot of exchanges here https://coinmarketcap.com/rankings/exchanges/
So that means you only trust few among the hundreds of exchanges listed in the market.

Bittrex is a good exchange too, I did not see it in your list, was it removed?

Bitsane appears to have exit scammed, so you don't have to consider that one any more.
