You submitted a share at difficulty 4 when the requirement was difficulty 2 (i.e. "4/2").  If the share difficulty threshold was set at 16 and you submitted a share of difficulty 36, then it would read "Accepted xxxxxxxx Diff 36/16 GPU 0"

"GPU 0" refers to your graphics card.  If you had two graphics cards mining, it would show something like "GPU 0" and also "GPU 1."  In other words, it's just indicating which graphics card was responsible for submitting that particular share.
