Just wanted to confirm that I also got burned for $100 by bitcoin4cash.  It's been almost three weeks since my order.

Has anyone ever filed an FTC complaint against the "Anti-Usury Agency"?

As far as I know, the postal strikes are still going on in Canada, so it could be months before the money arrives.

bitcoin4cash is definitely not the fastest service out there, but 3 weeks isn't that far from my own experience.  I recommend waiting another week or two.

Try and send the guy an IM...I found this info on his website:

"I'm The Madhatter (from the Bitcoin forums; username: madhatter2)."

Oh, and I'm in Canada too and I sent some mail to a customer in the USA last week, and it arrived already....
