Once a transaction has 6 confirmations, it is extremely unlikely that an attacker without at least 50% of the network's computation power would be able to reverse it.

Coin theft is often the case and it's very annoying in life, it's better to be vigilant in our coin safeguards, we should keep our keys properly and use 2FA's security to increase security.

Always implement second lock of account which is high security like 2 factor authenticatiin like google authenticator that gives code as a second line of security.To stay account as safe  to free from thief.

To prevent bitcoin theft we can use paper purse or also we can put our bitcoin in good market like bittrex and also poloniex, we can also add 2FA security code as security enhancer.

There are some way I use to be hacked you can refer
1. Use anti-virus software (Best is kaspersky)
2. When accessing the wallet, if you do not transfer coin, you should just use the wallet address to check the coin
3. Do not click into strange links, less search on google.
4. Mark your favorite sites like coinmarketcap, myetherwallet to avoid false links
5. Save the privatekey in the machine should be unrelated file name like doreamon, songoku ....
+ should add numbers like 0000 in the private key wallet
+ should compress the private key file and set the password

There are some way I use to be hacked you can refer

Or just use a hardware wallet and you don't need to save/compress/password protect anything on your PC. Just save your seed for the wallet and you'll be able to recreate your wallet in case of need. It's easier to protect your seed, if you don't have an electronic copy of it, just use paper and pen, write it down, make several copies and store them safely. You can laminate the paper if you want, to make it more durable and you can use a cryptosteel kind of solution in case you want to prevent it from fire too.

The simplest thing is to use an encrypted hardware wallet, such as a trezor or ledger. Also, don't store your seed in your house, memorize it thoroughly so nobody but you can ever access it. Also, you can use bitcoin offline, that way you're less vulnerable to driveby attacks.
Hardware wallet is what I would recommend as well. Though memorising seeds is hopeless, Our memory is hopeless. So, Don't memorize seeds, Write it down in a piece of paper and store in a safe place, Multiple copies at different locations.

Whenever any theft happens it comes to the fact that either it was due to malware downloaded by the user unknowingly or by phising attack when the user visits a phising website and provide its credentials.

So to keep yourself safe always try to use a separate computer for doing your crypto thing

Always use extra security like 2fa

Use offline/hardware wallets and store maximum funds there

Only keep minimum amount in exchanges if needed.
