So if I saw there were 15 entires, and I sent 16 immediately, would I technically be in the shoe for #16 since it also counts unconfirmed? Also you take this out yourself?

Whichever entry the game labels at #16, I will enter the game at the same amount.  This helps move the game along, and helps the #16 person get paid, but it does not guarantee it.  Does that answer your question?  If not, please elaborate.

Mine usually show up unconfirmed

It shows up with a # with 0/unconfirmed.

But the #16 person doesn't get anything directly.  It's just a way to assure them they won't be the last bettor, that they still have a good chance of getting paid out.

It also helps assure the #14, and #15 people get paid out.  People know that if we get to 16 bets, there will be a 17th bet.  The more bets, the more people get paid.

Nothing is guaranteed, though.  That's why it's gambling.
