In USA, Coinbase instant.  It will take you about 10 days to set up, but then you can buy instantly.

Not necessarily.   They will let you buy a small amount instantly until you wait about 30 days.

Check out www.virwox.com you can use cc to buy although fees are high and you have to wait 48 hours for first BTC withdrawal.
