Transactions must be included in a block to be properly completed. When you send a transaction, it is broadcast to miners. Miners can then optionally include it in their next blocks. Miners will be more inclined to include your transaction if it has a higher transaction fee.

Yup.  If you are in the U.S. they sell them at every major grocery store, in the little kiosk with all the gift cards and phone reload cards and crap near the lottery/cigarettes/checkout area, usually.  Just look for MC/VISA logos hard enough and you will find it.  Usually costs $5.95, but you have to read the fine print for the maximum value that can be stored.  Can range from $300 to $10,000.  My local stores don't carry about $500 max value cards, so for those might have to shop around.  Also not sure how to get the dollars from the exchange to the card directly, never actually did it myself.
