I've been running this for 2 months, but haven't been able to get it above 500-550 kH/s.

I'm currently putting the blame on the CPU now (Pentium 4 2.8 ghz, 4gb ram, non-SSD hard drive Smiley

i'm guessing the bottleneck lies with the CPU (only 32-bits).  is this clearly the issue?
or my settings.

I don't think it's the CPU, haven't heard of one bottlenecking a system. Hard drive speed has no bearing on performance... Post your bat / config

I have the same card, and the same problem as you
I am also struggling getting it over 500-550

as soon as I set intensity higher as 13 I get hw errors?

Like the others said it is not your CPU.  Others with 6 GPU rigs have no problem running a low-end Sempron in them, I doubt that's issue.

I'm having issues with my 7950 also   Undecided  My intensity is at 20 though, anything less and my hash rate barely breaks 400...

OP, your actual miner config would be a good starting point for diagnosis.

32bit is a big problem though.
