You touch it for a second to log in, and touch it for 3 seconds to withdraw. It works fine for me.

How did you get the code? Was it a random free thing or because you're a good customer?

I think I must have passed some transaction threshold, or had a certain amount of money/bitcoins stored on mt.gox.

A two factor system. You need to have the yubikey and know the MtGox password. The Yubikey generates a code that is needed as well.

MtGox optionally sells them for people wanting better security for (what was it?) $25 or so.
