In this kind of situation, what I learned was we shouldn't based on Google forms since the registrants can put what they want to in that form. People, especially cheaters can copy and put credentials without restrictions, so it won't be a solid evidence. For me it would be enough if the Bitcointalk account have posted that address or if it's in his profile.
