Does anyone know how to fix this issue. This log file comes from my Innonsilicon A9 Zmaster.

Does this mean my hashing boards are broken?

The board is not responding, it is likely failed.

AFAIK if one board is damaged the other two will still hash unless the controller is damaged.

What happens when you power on? What lights turn on? Can you find your rig on your internet browser and if so what do the System setting say? Is it a new unit or used? How long have you had it and have you mined with it successfully?

If you want help you need to provide as much detail as possible.

Show a complete boot process log, but that usually indicates non-detectable hash board.

The boards turned out to be faulty, I had to send them back to the manufacturer and replace them with new ones or should I say used ones, Innonsilicon sent me used hashboards as a replacement, what a joke. So much for the guarantee.
