It seems that you might have two bad condition hash boards.

Do you still have warranty for that T9+?

Make a support ticket at Bitmain website.

did this happen after a firmware update or just out of no where?

first 2 things you should always do are

1- reset miner.
2- change/flush firmware.

these are the likely fixes for this issue.

if it fails, then

3- try to reduce the frequency, some boards when over heated and over time tend to only work on certain lower frequency.

4- try another PSU  " very unlikely to be cause"
