I have tried several ways to fix that: 1- 2.5 amp power input for the controllers 2- 60cm shielded usb cable 3- updating the controller's firmware to the latest one 4- less chains and less miners in each chain but the problem is not fixed. Am I missing something?

How many miners per controller?
What is the power brick for the controller?
What is room temp?
What is internet service.
Also both mcu and rasp pi must match and be up to date.

Try one chain at a time in each USB port on the pi, it's possible the pi has a bad port.
If none of the ports are bad then check each AUC, we had a bad AUC once that would keep restarting.

Do port 1 with 1 miner Do port 2 with 1 miner Do port 3 with 1 miner Do port 4 with 1 miner
Same miner same wires.
If it works on all ports.
Do port 1 with 2 miners Do port 2 with 2 miners Do port 3 with 2 miners Do port 4 with 2 miners
If it all works
Do port 1 with 3 miners Do port 2 with 3 miners Do port 3 with 3miners Do port 4 with 3 miners.
Yeah it sucks as it is slow.
Once you now you can do all 4 ports with 3 miners 1 port at a time.
Try 2 ports with 3 miners Try 3 ports with 3 miners Try 4 ports with 3 miners
Very often chains of 4 and 5 fail and reboot.

He doesn't need to do all this work if he knows it's always the same chain, he can flip on the led to find the chain that keeps going, then use the known good chain to check the usb port.
We had one pi that would keep dropping the chain it would usually get up to AUC# 8 within 24 hours, quick swap of the AUC and it never did that again.
These instructions would be to try and locate a bad i2c cable, which it is possible to have a bad one but based on what he's said and posted if it was a bad i2c cable it would likely be the one connected to the AUC.

I've always suggested people to only use 2 or 3 per AUC3 chain, i.e. 8-12 miners per RPi so that stales don't get too high More stales means more lost shares and more possible lost blocks. I'm not sure how bad this is with the A921, but certainly was necessary with the A7 and A8 YMMV
If you can't find a cable problem, get another RPi for half of them.

my instructions will find
a bad port a bad cable a bad auc
lastly my method like kano's advice is chains of 3 are really good and chains of 4 and 5 are iffy
he said he has 50 units 4 rasp pi with chains of 3 will do 48 miners.
he would have 16 chains of 3 he could play around with the odd 2 miners to see if he has a good 4 unit port
actually he says all 50 fail which more likely means a bad internet issue
modem >router> switch >
>>>rasp pi >>>rasp pi/canan >>>rasp pi/canan
the router to switch connection could have an issue the switch could have a problem
he has multiple places to check at op good luck doing this.

Weird, I have more than the recommended amount and am under 1% bad shares. When I didn't set a custom difficulty for the pool this was much higher. Very often chains of 4 and 5 fail and reboot.
I have more than this and only have seen 1 AUC cycle, it was a bad AUC it didn't work with just 1 miner on it.

Are they 921 or other models? May I know your setting? Firmware, controller, and usb cable? I read somewhere that usb cables should be fast charge, is it relevent?
Update: after several attempts on one controller I've got 3-3-3-1 stable miners with no error. It's still not possible on the other controllers.

Re USB cables, yes they need to be fairly short - as in 1M long or shorter - and preferably good for 'fast charge' which implies heavier gauge wires in them. Also relevant is that they be DATA cables and not just charging cables. In your case we know they are data because they *do* communicate. Something I've yet to see you post is the CGminer API log. That is much more useful than the kernel log as it tell us the condition of power in, Vcore, and AUC details and much more... The miners mine for a while and suddenly a chain is reseting due to usb disconnect error.
I've tried different controller's firmware with different MM versions and still the same problem exists. However, as I can see in your log your MM version is MM ID1=Ver[9211809-b4b3d00], which I didn't find in their repo. Would you please send here your firmware's .mcs file?

Hello there I've been facing such a this crazy problem for 4 months. It's got me insane exactly. I've done whatever need to be rechecked. Controller ampe changing DSL Using stabilizer ,...
but still restarting. But I have an error on my ECMM[128] or 130 and I don't know what does it mean?
