The Chain[5] is not running properly which might lead to the other two hashboards stop running.

Can you try to remove the first hashboard and test it again if the other two hashboard will run without any issue?

You can just remove the 6pin 12V and the ribbon cable from the first hashboard then run it again. Let see if you still see some error related to PIC FW.

let me ask again.

do you only have 1 s9 the broken one?

or do you have a few working s9's?

controller could be bad = very likely
firmware in controller could be bad = possible
boards can be bad  = very likely

eth cable bad = maybe
you filled the pools out wrong = maybe
bad psu = maybe

all 3 communication cables from controller to boards could be bad. = not that likely
pool connections could be bad = not likely

If you only have 1 s9 with 1 psu it will be hard to troubleshoot.

i have 36 other S9s. 3 of them just started having this problem. I tired to swap to an apw5 from an apw3++. still no luck.

Once you have long passwords on the good ones.

take a good controller and switch it to a dead s9

if the dead s9 works  you may have 3 bad controllers.

but get a good password on the s9's that work.

This is most likely and issue with the hex on the hash boards, you will need a special tool to fix the issue, more details can be found here, the most common reason for this issue to happen is a bad firmware, you have not mentioned a thing about it, but I am pretty sure you have tried flashing a firmware and got your miners f***, if it was not you, then probably a malware did it.

If you are lucky enough, a different firmware might solve the issue, since every firmware deals with the hash boards in a different way, you might get away with just flashing BraiinOs or Vnish aka Asic.to and AwesomeMiner, so before purchasing the tool/s try a different firmware.

Your miner is fixed frequency, but not set freq on your config files ! (problem)
on your chain 5 , first, have problem with pic firm (need pickit3 for resolve)

For resolve this, unplug your chaine 5, after you need reflash good firmware, this is perfect S9J but compatible with all S9 / S9I / S9J (https://file12.bitmain.com/shop-product/firmware/Antminer%20S9j/Firmware/00720181108111659649oC0LlMOk06A7/Antminer-S9j-all-201811071118-autofreq-user-Update2UBI-NF.tar.gz)

Flash this, without keep settings.
