Yes, that's the worst-case scenario. No reason for Inoue to fight him now if he is has a medical condition that he can't pass through. Aside from that, no title to unified now as Casimero will now be a normal challenger. Inoue will end up instead facing Butler or Donaire.

But as you said, there are surely things that are done now to make sure Casimero is in a good health condition at the time of the fight or even prior to weigh-in. Quadro Alas should already know the drill and working on it.

The win against Butler might trigger the interest of Inoue again to fight Casimero. If he stays healthy throughout, we can still see him facing Naoya Inoue. I don't think Inoue is avoiding Casimero. There's just another big opportunity for him instead of facing Casimero. Not unless Casimero will be able to win all his upcoming fights until they negotiate a match again with Inoue.

If that's the case then Casimero should accept and sign the contract at once, he's already that lucky because Sean Gibbons himself is making some efforts and extending his hands just to get Casimero under the MP Promotion's roof. Besides, MP Promotions is the best choice for a Philippine boxer to get a chance to fight bigger names in the industry.
