Normaly you have to write to the exchanges and ask them if they can or add your coin to there Exchange.
Mostly there will be a Listing fee for coins at a specialy rate , its diffrent from exchange to exchange .
Others have a voting system where the community vote for Coins that maybe getting to the exchange.
Also for mining pools its the same you have to ask for it , if no pool already added your coin.
