Exactly the problem with some people, some people knew they are addict but they won't accept that fact because sometimes it will be like a disgrace or shameful thing because addiction is not a good thing but instead of denying that fact and making your problem increase why not just take away the shame and accept the fact so people can help, because if someone doesn't accept or admit they are an addict there is nothing those who want to help will do. I think gambling addiction affects the mindset of addicts, some don't admit the fact that they have gone overboard while others that acknowledge that they have a problem don't know why they keep going back. Moving is a working solution. I had a friend who was addicted to gambling, and he had the opportunity not just to move, but to go live and work in another country, and you know, it really worked. It more about the discipline and knowing what you are doing, if you know what you are doing you have the knowledge knowing that gambling is not the way to wealth and its not going to make you rich, there are probably some people that about it earn a huge amount of money on gambling but I could argue that the money is already lose because they doesnt have the mindset and it will not make them rich, because they dont have the mindset of the rich.

What you have just mentioned now is the main reason why many people's diseases go untreated for years. People not coming into terms with the reality of life around them as well as them not being able to face their fears by working on their weaknesses. Okay, this is a solid point, because environment can really feed addiction. If someone is always hanging around betting shops or centers where they are known, allowed to book on credit, or even encouraged because of customer relationship, it can becomes very hard to stop. Changing location or breaking away from that circle can reduce the urge with time, especially for someone who is not financially strong and depends on those connections.
