1. Depends on where you live because you can often get less fees and better, faster support from a more local exchange. Personally I use p2p exchanges where you physically go to the office and exchange money. I don't use exchanges, but I've used Coinbase and Binance a few times without issues.
2. Right now probably Trezor is the most reputable. It used to be either Trezor or Ledger, but since Ledger chose the cloud route, there's only 1 OG player in the game. I've also heard a lot of good things about Blockstream Jade because it allows you to scan QR codes and still remains cheap.
3. You can choose any hardware wallet and achieve the same result.
4. The wallet only holds a private key that allows you to move your coins from one address to another. Blockchain has never been hacked. If you want to read about possible angles of attack read about double spending and 51% attack.
5. probably brain wallet because it leaves no trace and only you will ever have access. The way it works is you learn the seed, but there are obvious flaws, like brain injury from an accident, or mental illness. I'd say that second best way is a well stored physical seed, like Cryptosteel or Cryptotag. These are almost impossible to destroy and can be placed in a hole drilled in a wall of your house, or inside a curtain rod, which makes them impossible to find by a thief and fireproof.
