It depends on the minimum threshold. If it is small, then I behave like you, I think: I simply make an all or nothing bet and that's all, I don't think about it too much or give it much importance. But if the minimum cashout is high, like in a few casinos I like to play, then I deposit the missing part and play a relatively safe game, like dice with >90% chance of winning, and then I withdraw. This is very simple.
There are two things I would do if my bankroll fell below the minimum withrawal amount. First, my plans was to gamble with only the bankroll, whether I lose or profit in the end. I wont exceed my bankroll. What I would do is to keep on playing till I win or lose. The other option is to deposit part money so that I can withdraw it. That is, if I no longer want to lose what's left.

I gamble in two ways - sport betting for profit and slots with minimal bet for relax.
In sport betting i mostly withdraw all money after win and deposit before event, i plan to bet, so i don`t have such a problem here.
When i play for relax - i just don`t withdraw money. If i win - i leave it on bankroll for next game. If i lose - ok, i deposit some sum. But i don`t change bet size and i don`t change my rules to lose it faster or get sum to withdraw.

I'm always intentional and careful with my bets regardless of the odds of winning so whether I'm below minimum cash out or not I will try to increase it by reducing the risk which means that I will select few games that won't give me high odd because most times the higher the odd the higher the chances of losing or the lower the chances of winning and sometimes I make sure not to go below minimum cash out because sometimes increasing it can be very difficult unless you fund your account.

I rarely withdraw money from a casino. What I do is manage my bankroll well so I can have fun for as long as possible without aiming for profit. I do not know... maybe I am wrong, and I know most people will say there is no such thing as "playing just for fun", but I identify with gambling in that way.
