I am looking at the fields that need to be filled in on Bitcoinica, and they do not seem to match what My bank has given me for inbound wires.

Has anyone mastered this process?  Can you lend some advice on what goes where?

Bump.....and adding a 1 BTC bounty for someone affirming they have recieved a successful wire, and indicating how to properly fill out the wire form.
