The foot cutting scene in saw by Lawrence Gordon, i was eating back then and puked all over the keyboard and front desk and the human centipede it was the worst.
Oh shittt I forgot about the human centipede, yeah that was seriously fucked up. The part where he "feeds" them and then the guy has to go to the bathroom eventually... really disturbing.

This goes way back to Canadian television around 1970. First topless scene ever on prime time tv. Show was called "The Manipulators".
Gathered around the television with the whole family and this bare bosom appears. Silence and red faces everywhere.
On that note, if we're going to take context into consideration, I remember one time when I was maybe 12 or 13 and my family had my grandmother over for dinner. My sister picked up a movie for us to watch from Blockbuster. Well, she selected a comedy that turned out to be a really dirty, raunchy movie and, while fucking hilarious, it was incredibly awkward!!! I can't remember what movie it was though.

Most disturbing scene I've *ever* seen on TV was the one from Little House on the Prairie when there's the fire at the blind school and you see someone trying to escape with Mary and Adam's baby but they can't make it out. In fact, that scene put me off that show forever. I can't watch it anymore. That scene was just very graphic to me. Normally, I can eat while watching Bones, CSI, etc. - very little bothers me. But that..ugh. Just ugh. It's on Youtube if you haven't seen it before and want to be scarred forever.
Yeah, I never understood why she just didn't throw that baby out the window to the people waiting below...

The Cannibal Holocaust was extreme. There was a scene in the movie, where a tribal girl is girl impaled on a stick after being raped by some foreigners. It was really disturbing.

I think most disturbing scenes in television shows or movies are the adult or crime scenes.

A Serbian Film is obviously a very insane movie, but definitely has it's charms. The feeling and looks are great with very good acting.
The baby rape scene, the main character raping his own son still sickens me to this day.

The film is just meant for people who are mentally deranged. I don't know why people are praising that movie as a cult classic.

Ever heard of a movie called ABC's of death? You should watch it if you're a fan of horrors! Here's the scene I found to be the most disturbing: https://www.youtube.com/watch?v=-DmL8-nGCEA

Prior to puberty, the whorehouse scene in the barn stalls (seriously) in the very first movie I ever paid to see with my girlfriend, later my ex-wife: Ode to Billy Joe. I couldn't figure out what they were all doing and it scared the shit outta me that all movies at theaters may like that one.
The eating of live monkey brains scene in Faces of Death, also comes to mind.
I still haven't figure out Body Double, with the ending way too weird.

I actually don't find a lot disturbing these days but Game of Thrones red wedding scene was a mindfuck for me because they normally never show that much death and blood even on those stupid police dramas, I'm also not particularly happy with the news because whenever there's a murder story or war going on in the world they have a disturbing fetish for either showing photographs of absolutely everything to shock you on purpose or they make these unnecessary dramatisations.
Actually, nevermind, Game of Thrones is tame in comparison to that, because at least the purpose of it is to tell you a story, the news just use it as a method of trying to shock you.

I'm not squeamish when it comes to movies, there's always the "it's just a movie" thought in my head but the head-smashing scene with the fire extinguisher in the movie Irreversible was very realistic. Brutal.

The scene from the movie Michael Clayton where the two hitmen paralyze the victim but leave him conscious while they inject him with an overdose. It was just so creepy.
Also, a short documentary I watched about how animals are treated in China. They apparently skin them alive because it comes off easier that way. There's this one shot of a poor little thing with all the skin peeled off its face but its eyelashes somehow still attached, just lying there panting and blinking and looking pitiful on a pile of others just like it.
The Red Wedding was pretty shocking. Especially if you're too stupid to figure out why it's unusual for someone to be wearing chainmail at a wedding feast.
