Probably Cannibal Holocaust.
The Human Centipede also ranks pretty high.

all these are high rank disturbing for a miscellaneous weirdness lovers i would mention gimmo, a serbian film, pink flamingos, antichrist; for more necrophiliac taste: nekromantic, Aftermath; for sci fi genetic taboos: Splice

Requiem For a Dream was pretty weird for me, but might be normal for others who have used before.

I start with Eraserhead
Oldboy (2003)
Both excellent films, disturbing and confusing, yet gripping. I will nominate in the serious category: Irreversible The Ring (original Japanese version) [REC] (2007) - yeah it's not exactly original, but something about it really made me very jumpy... Weird psycho-comedy category (obviously all Japanese haha): Visitor Q Tokyo Gore Police Tetsuo: The Iron Man Hausu!!

Nothing is more weird than David Lynch. Twin Peaks watch the whole series in one sitting and then ask me "Who put the acid in my popcorn?"

The Human Centipede A mad scientist kidnaps and mutilates a trio of tourists in order to reassemble them into a human centipede, created by stitching their mouths to each others' rectums.
