Not a single investor/buyer would join in the 'scam'' as they would notice the hige premine and ignore it. Better off thinking of a new concept and releasing it after developing well,

there are nearly 2000 coins from that site alone ...even more shitcoins out there than that. without anything to make it unique, nobody will care or waste time mining (or trading for) a clear scamcoin

Basically yes, it could be simple copy paste. Even easier to pick up the source code of an existing, feature rich coin what looks the most appealing for you, rename it, change the images, maybe you will need a bit of tinkering with the emission rules and then you are ready to go .

As far as I remember, I read that Bittrex charges a fee of 5 BTC unless they like your coin and add it for free.

Yes, exchanger fees are the more pain-in-the-ass part of creating a new scamcoin. Cryptorush used to ask something like 0.5 btc, but now we all know why.
