1. Is there a monthly fee? Amount?
2. What is the maximum amount to be loaded on the card? (I assume none but still $1,000/daily ATM withdrawal.)
3. Is there a weekly or monthly ATM withdrawal limit?
4. How is a purchase made? Do you provide a URL where it is automated?
5. How is the card load done via bitcoins? Is it automated; URL?
6. How long after a load via bitcoins does it take before funds can be withdrawn via ATM?
7. If I ordered this for a client, can the card be in name A and shipped to name B?
8. If two cards are ordered and shipped to the same address can they be in different names?
9. Can the ship to name and card name be in an entity like a LLC?

2. You are correct, just daily limits for withdraws/purchases

4. Purchase can be made through Liberty Reserve and Bitcoin once you contact us. Purchasing and Funding will be two separate things.
5. Yes, you will use an automated page that will then confirm the deposit. Once deposit is confirmed our server will connect to the bank api and credit your account from our master account. This all takes between 5 and 10 minutes

8. Yes, the limit is two per household
9. Business names are allowed.

The card is shipped directly from the bank either regular mail or courier. ($29 or $49)

I think what he intended to ask was: Can a single BitCoin address be used to fund a particular card? This is similar to how many of the exchanges, etc. work. Anytime bitcoins are sent to address X then it credits account Y. In this case, could bitcoins sent to address A always load card B.

Yes exactly. What I was asking is can there be a single address that I can send all my Bitcoins to in order to fund my account? That would be useful in a sense that I can always have my mining profits from a pool sent to my card.

That is possible because funding by bitcoin is unique to each address.

It's through an offshore bank, additionally there are certain countries we do not ship to and that is obvious why.
