It's Diamond Brand. I just bought it brand new 5/13/11.
My friend mined with it in his computer for <1 day. I'm still setting up the computer that I was going to use to mine with it.

I've managed to get 820 MH/s out of it now, so I'm going to hold on to it... I apologize for any inconvenience, and with a couple more cards added to my rig, I may end up selling, but for now, I'm going to keep it.
