Antminer U3 runs for a few seconds before Icarus erroring

After about 30 seconds to 2 minutes of running my new Antminer U3, I receive one of two errors. Either 'Icarus detect failed to initialise' or whatever this is.

Do you have any idea how to fix this, if you can at all? The miner runs perfectly for a short time before this.

Very similar error.  Running cgminer 4.9.0 using the following .bat file: cgminer.exe -o stratum+tcp://stratum.bitcoin.cz:3333 -u user.worker --au3-volt 750 --au3-freq 225 2>logfile.txt  

Needed to follow the instructions explicitly as directed by Antminer - Downloaded cgminer-run-windows....rar from Antminer. Created .bat file using: cgminer.exe -o stratum+tcp://stratum.bitcoin.cz:3333 --bmsc-options 115200:0.54 -u "user"."worker":"pw" --bmsc-voltage 0800 --bmsc-freq 0982 2>logfile.txt  

This is version 4.6.1

Running at 45 Gh/s (I have a very slow internet connection - may be the issue)

Don't touch the USB cable - causes errors!  I need to change out the cable perhaps.

Miner and power supply running slightly warm.
