edited the index.php to enter mySQL data

however, this is what happens when the you go to the page

what am I doing wrong?

I hosted it on 000webhost just to test the script

Well, not to sound like a dick, but if you can't set up a small script like this, I wouldn't be attempting to host an online service. It is a recipe for disaster. But, if you must.. Google is your friend. Almost 99% of errors and problems, their solution can be found by googling, even if the scenario is a bit different.
