At least where I live it is very easy to get anonymous pre-paid SIM card for phones. Also teh SMS stuff isn't required, it is just made to make the whole process easier. You don't have to use it. You can just rely on emails.
Anonymous SIM isn't enough. You want a clean phone, too.

I don't know much about this, but what do you mean with a clean phone? Last time I bought an extra phone with 20 eur, cash, from a small store, Germany. No questions asked. I see no way to trace the phone back to me. But maybe in some other countries they ask papers even for phone purchases?

Is it possible to make a service like this? 1) you make the account on the service 2) the service gives you code (example: 34757893368) and a phone number. 3) you go out and buy a phone card ( it's usable on phone boxes ) 4) you insert the phone card on a phone box 5) you call the phone number given by the service 6) you insert the code: 34757893368 7) the service will instantly empty the phone card and will convert the money in bitcoin on the account.

I don't know how it works out-site of Italy. But here we can buy phone cards that are usable on phone boxesCards are in FIAT money. The service must give a phone number that the user call from a phone box with a phone card until the card is empty. After this the service will convert the money earned from the call in Bitcoin in the account of the user.

I used localbitcoins.com last week (just after they added escrow and SMS services), and it was really a smooth process. We settled the issue in the street in a few minutes (because it was the first time using localbitcoins for both of us).

I'm assuming both parties have their netbook or laptop with their BTC client, you see the cash, you send the BTC? They verify the incoming transaction then hand you the cash? Each with their own PC and BTC client would be preferable to avoid suspicion. I wouldn't trust someone merely showing me a block explorer of my address and their transaction...not on "their" PC at least.

No need. Look on their site: you just need to be able to send and receive SMS. The seller sees the money, sends a SMS to release the escrow, and the buyer receives a SMS saying that the escrow has been released, exchange is confirmed and money transferred. Buy something with bitcoins that you can easily sell (such as a GPU) and then sell it for fiat.
or gold from coinabul. Any coin or pawn shop will take them at spot. Most coin or pawn shops will pay less than spot.

Plus when you try to sell anything to pawn shop they will photocopy your ID. Gold coins from coinabul than sell on craigslist privately works well though. You will get a better price as well. If you price match apmex it's a good deal for both parties (no shipping costs, anonymous)
