Most likely an inside job.

In the event that it was not an inside job, what is with people who have no clue about security making exchanges handling millions? With all these hacks since 2011, a bunch of people are now millionaires from just learning how to code and hack as a result.

I wonder how did a cold storage got stolen. And also a comment on the reddit post "This reminds me that Bitstamp still hasn't provided any kind update as to how their hot wallet was compromised last month"

A cold wallet supposed to be "disconnected" from the internet, I'm still thinking how that is possible.

Clearly, no other explanation. It would be very difficult to trace though as the hack would have been done by someone external acting on the insider information.
