I am very curious to know what the unique circumstances are. I would think that a moderator's main account would generally be associated with the fact they are in fact a moderator so they can be held accountable for their actions by the community when they actually act outrageous in some regard

I think it would be much more difficult to do so without knowing their 'main' account. Moderator's actions are by default anon as you do not know who has taken specific action against specific users. It is only over time when trends start to occur that you can potentially complaint about actions
