I too received the email (went into Gmail spam, with a note about being flagged as spam by other users but nothing about phishing). Just like others have reported, as far as the headers are concerned, it looks like it was sent by the legitimate Coinbase servers.
 - The email address where I received the message was "leaked" by a stupid Bitcoin-related service some months ago when they sent an email to all of their users and put everyone's email in the "to" field (endless spam since then);
 - I don't have a BTC-e account with this email address;
 - I have a Coinbase account, with this email address, which I created for the sole purpose of receiving the free BTC they were giving away at launch, and is abandoned since then;
 - I had a (never used) MtGox account on this email address.

Since there are people reporting to have received the phishing mail on a address used solely for BTC-E, but I don't have a BTC-E account on the address where I received it, probably whoever sent the emails is using a list built from various sources.

I am experiencing the same situation. DNS hack/configuration problem?

Care to explain why the minimum wager for payout for people who received the free 0.01 BTC has been increased from 1 BTC to 2 BTC? There's no way anyone is going to play on that platform if the rules keep changing without previous notice. Reaching a total wager of 1 BTC is already hard. If you used that password on another site or you logged in to a fake Mtgox site thinking it was the real one, no one had to crack anything to get your password.

Visa is not a currency, just a means of payment / moving currency. Visa does not issue currency. Bitcoin is a currency which happens to implement the means for its issuing and moving between "accounts" (addresses).

I have the blockchain.info app installed on my Android device, but I am sure that I never created a new address within it and I'm also sure that I never created a transaction on that device. Basically the app only acted as a way to check the wallet balance and transaction history (i.e. read-only actions).

Are my private keys and transactions at risk if I don't do a key rotation? With the many small and non-mature inputs I have on my many addresses, I am heading for maybe over 0.02 btc for transaction fees... last time I did a key sweep it was something like 0.01 btc, and to be honest I think my wallet is even more fragmented now.

I don't think the app ever had any reason to request random numbers unless it is creating addresses without user intervention.

On a related thought: many online wallets generate private keys client side with JavaScript. How secure is the PRNG used by JS, or is it not used in a direct way (are there other sources of entropy)?

And I couldn't understand from your posts, but does the problem occur with any USB sticks that you connect to that computer, or just with these two sticks?

Do you have any other USB hardware that puts stress on the USB transfer (e.g. HD-quality webcam), that you can connect to see if the problem is just with mass storage devices, or with any kind of USB devices?

Can you boot some Linux distro on these computers, transfer big amounts of data to and from the USB sticks, and see if problematic behavior occurs on Linux too?

I have heard some of his work on Jamendo, and it's not the kind of music I usually listen to. Looks like something that appeared back in 2010.
My guess is that you accessed some page with malicious javascript embedded, and somehow the thing managed to stay in memory until your next login to blockchain.info, where attackers could get the password and the encrypted wallet. Or it may have been a trick using iframes, not so sure if that's possible and how it works.
Malware that replaces the JS files executed by the browser when on the blockchain.info wallet could also do it (since the thing runs on lots of JS), but from what I remember blockchain.info implements file signature checking to prevent tampering (or am I confusing with a browser extension that actually does that?).
Or you simply got tricked into logging in to a blockchain.info site that wasn't actually the true one... but that wouldn't get them the encrypted wallet, and your username and password alone wouldn't be enough because of the two factor auth thing.

Just one thing... "PC" is definitely not a OS name and "Mac" usually refers to the hardware by Apple. "Microsoft" is not a mobile OS name either.
I think you meant "Windows, Mac OS X and GNU/Linux" in place of "PC" and "Mac", and "Mobile - Windows Phone/Windows Mobile[dead]" in place of "Mobile - Microsoft". Just thought that if you want to take this as a serious thing, it's better to pay attention to detail and be precise.

And as gweedo said, sqlite is definitely not a good choice if they are going to hold a big amount of transactions (much less the whole blockchain).

I think I just found a bug. I opened the page, the game entered "demo mode", and I entered 0.01 for the bet and a 80% chance of winning. The "if I win I will receive" box changed to 0,0123. When I click the Spin button, I get the following JavaScript alert:
