Once a transaction has 6 confirmations, it is extremely unlikely that an attacker without at least 50% of the network's computation power would be able to reverse it.

Maybe bad contact. Check all of ribbon cable, unplug the ribbon cable then plug in again.

Start out here - https://bitmain.zendesk.com/hc/en-us/articles/204875015-How-to-diagnose-S5-hashing-PCB

Assuming that does not help then go here  -  https://bitmain.zendesk.com/hc/en-us/articles/203559969-My-S5-hashes-at-about-600GH-S

And if that does not do anything then contact bitmain support.

I had that problem for a while I just made sure the firmware was up to date and after the firmware update it resolved the issue and now works great.
