Firstly: In normal operation the LED's flash green util they cycled the startup script and when all is returned well the will be continuously green. When some of them flash green and some are off, that means that the card that is off is defective somehow. I don't know what the red flashing means
Secondly: I assume you have switched out the defective PSU.
When I got my A2 mini I switched the crappy PSU for a silent more efficient one.
I noticed that when comparing the stock PSU with the PC PSU (I don't know if that is always so, but it was in my case) the polarity on the pci-e power cables (that go to the blades) were reversed.
Possibly that is in your case as well. If you still have the old PSU with cables attached to it, measure the polarity.
Thirdly: What software are you running? If you have old A2's you might try:
