There is a lot of possibilies here , I can't speak about Xapo and E-coin cannot be shiped to U.S if you are there unless you ship to a friend elsewhere the nhe ship it back to you . Bit-x.com seems to be good and also they removed the monthly fees as far as I know , same goes for Advcash they have low fees and also low cost (if they didn't change the prices then it's 1$ for Virtual carda nd 5$ for Physical card)

I have a Bit-X debit card so I can tell you a bit more about. It works well, you can withdraw Fiat on any ATMs, also pay everywhere. You won't be able to check your balance though, at least I couldn't find one to do it.
Fees are steep, very steep but this is to expect since the service is somewhat new. Fees will probably go down as more competition rolls in. They have already dropped the monthly maintenance fee no matter are you using your card or not, which was a major drawback anyways.

The cheapest card at this stage would be Xapo but they do not ship to the US.
Its also, AFAIK, the only card that do not have limits each year and or life-time limits [after the full KYC story].
Bit-X must be the most expensive card at this stage with their 0.0002 btc per transaction plus another 1% bank fee on top of that. The card has very low limits but you can bump it up two notches but the limits are, in my opinion, still to low then.
Both of the above cards work directly from your bitcoin wallet so there is no need to 'load' them, you just swipe and the funds are taken directly out of your bitcoin wallet.
The E-Coin card fees are okay but you sit with a loading fee to load the card and that adds to the cost which also makes this card more expensive than the others. I cannot remember what that fee was now but I know that made me lose interest in the card as it drove the cost up to high.
I know about some other cards but do not have enough information about them. In my opinion the Xapo card is the best at this stage.

I haven't used a Bitcoin debit card but I know several companies are offering theirs for free - personal use is more valuable than any recommendation so why don't you test a few out and see what YOU think. It's easy to set up a wallet, fees are minimal, transactions don't have to be high. Just get a few of these cards and then use them to buy a coffee. Let the rest of us know what you think because it would be very valuable to the community here.

Then I'd suggest you E-coin.io , it only costs you 17$ and I guess 12$ if you use someone referal link this is for the Physical card . for what comes to the Virtual card (you can use it to verify Paypal or other websites and also use it to buy online) and it's free . 0$ you just deposit your BTC to your E-coin account then load the Visa card with it .

You can check your balance right in your balance page (when you're logged in)! It'll show you the maximum transaction amount under 'Your Bit-X Cards'.

I would recommend either Bit-X or Xapo. https://bit-x.com/cards or https://xapo.com. They are both true debit cards meaning you don't have to preload them, just send BTC to your deposit address and you can spend it through your card aswell as through the site (Xapo: Send it as BTC; Bit-X: Trade, Buy GH/s or send it as BTC).
