To get mine working I had to do each card individually.
Disconnect everything
Start at the first PCI lane. Plug in a card, connect card to monitor etc. Boot up, let it run through. It'll black screen a bit, delay, restart, as it's setting the driver for that lane. Then it'll boot up fine as per normal. Switch off, disconnect this card and connect the next card into the next PCI lane.
Repeat this for each card.
**Make a note of whats going on at each step and it'll help you, and possible us, troubleshoot the issue.**
I had the same issue as you and doing this I found out that two duff USB cables in the risers and managed to get all 6 cards operational.

Same problem with the MSI Z170-A PRO. Only 4 cards are working. 2 cards code 12. Has anyone found a solution to this?

I had the same issue as you and doing this I found out that two duff USB cables in the risers and managed to get all 6 cards operational.
Wich mainboard and wich cpu?

In the BIOS, maybe you can change the PCIE generation from auto or 3 to generation 1, that might help.

Same problem here - can see 4 out of 5 on ASROCK B150 PRO4/D3, Win10. Connecting a fifth gives me THREE cards with error code 12. All cards are fine, all RX 400. Turned off all the non essential devices, upgraded bios to latest switched to pcie gen 1(tried gen 2 as well...also noticed in the new bios there's actually separate setting for each pcie slot), disabled integrated, including gpu multimonitor (btw with integrated graphics on - the system won't post) tried all on USB raisers and with one card in slot upgraded win to latest... what else... contemplating reinstalling windows although doubtful if it will help...
kinda out of ideas ... will go for a while with 4 ... maybe later will play with power/raisers.

had the exact same mobo, tried *almost* everything, gave up and returned it h81 pro btc working smoothly oob I found that the H61 BTC pro or the H81 BTC pro are the best mother board out there. They are very reliable.

I have Asrock H81 Pro BTC, chipset Intel H81, and only 3 GPU and the same error. The sistem work with 2 GPU, but CODE 12 error with 3 GPU. https://i.imgur.com/7wiGECO.pnghttps://i.imgur.com/NbpYMjH.pngIt did work sometimes after several restarts. I tried on Ubuntu 14 and the same. The system see only 2 GPU, so I belive it has something with the BIOS.

Using 1x to 16x powered risers right now, will be using usb 3 risers next week.
I have my rigs (3) running with only 3 cards now untill there is a solution. I hope someone can help us!!
edit: Just got 4 cards to work, dont know how/why.

Asrock z170 extreme 4 i3 skylake not working ... changing to asrock h81 pro BTC ... but i think the problem on z170 is the CPU skylake not the motherboard )

I just set up a mining rig with Conf : - Intel i3 - 6098P CPU - Asrock - Z170 Extreme 3 - AMD Powercolor RX 480 GPU - 6 gpu - Kingston 120 GB SSD - Corsair RM 1000x PSU. - windows 10 Enterprise x64 - Ram DDR4 HyperX Fury 2133 Mhz The recent problem I encountered is when joining 6 gpu's. 3 of them are running perfectly fine and in device drivers window showing - "This device is working properly". The other 3 are showing this in the driver - "Code 12: This device cannot find enough free resources that it can use. If you want to use this device, you will need to disable one of the other devices on this system". http://imgur.com/a/MhWI6I have tried my level best to work on this error but unable to solve. when I connect 5 of them only 2 runs fine and the other 3 of them doesn't work as the same way above. I have tried solutions given by microsoft and other aslo. but no clue still. Kindly Help. "When connecting 5 GPU. 3 of them shows the Error only 2 works. While connecting 6 GPU 3 of them shows the Error only 3 works. When reconnecting 4 GPU all work fine". HOW ? WHY ? You should have maybe read through the forum for compatibal boards before you purchased the components. None of the skylake boards (B150, H170 Z170) work with more thsn 5 GPU. Most will work with only 4. There is no solution, this is a hardware limitation. If you want to use more than 6 cards, use an older motherboard/processor/ram combo, like the several mentioned in the replies here. If you purchased 6 cards, two are now useless and its best you return them. If youre in bangalore I can buy them off of you.
