I don't know if is work in VPN but you can try to find out if is works or not.

It will not work on VPN because it requires to submit a copy of your insurance card as a proof. You can fill the survey via VPN but how would you submit the card

Interested in trying this out, Has anyone else given this a shot yet ? Any risks I would be running by giving up my current insurance provider information ?
