MOSCOW, December 1. /TASS/.

Russia’s cargo spacecraft Progress MS-04 has most likely entered a wrong orbit and will be unable to reach the International Space Station, a source in the space rocket industry has told TASS.
"The most negative scenario is most likely and the ship may be lost," the source said.

This will be a serious blow to the Russian space program. During the past 2-3 years, a number of the Russian space launches have ended in failure. They should seriously look in to this issue, and rectify the issues before it worsen any further.

They can't do anything about it. After Ukraine ceased to cooperate with Russia in space industry Russia has no chance. While Russian ships Ukraine provides fall of her feet for engines European countries and there is no problem.

Accidents do happen when you're dealing with such volatile mixes of chemicals. This is part of the reason these rockets are so expensive, it's always a shame to lose one but it's fantastic they are built in the first place. Spreading into space is the only way for humanity to survive long term.

I do not think so. 

Everyone has to accept one in ten rockets blowing up.  Or whatever the percentage is.

This is not new.

Russia has an amazing record with Soyuz.

That is one big of a failure. At this point russias space program will be a laughing stock by other countries space agencies. They not only make their program a failure but wasted millions of money and resources just to end up a failure. But this will serve as an instrument to make the next space program more effective, they should better learn form their failure.

But we don't have anything else but this old tech. Current rocket tech is ~90 years old.
Without some kind of groundbreaking scientific breakthrough there is little hope for human race to ever reach the stars.

Russia will never recognize their mistakes. In addition, Russia has still not realized that she is much weaker than was the Soviet Union. Without Ukraine space program would not exist, and create a new they do not have the strength.
